Physician Pending-Members and Adjunct Pending-Members:
A special code was emailed to you. Please enter that code into the field above, click 'Submit', and then select the appropriate registration type from those displayed. Contact the ISHRS HQ if you have not received your code.
Physician Non-Members:
Please fax the appropriate registration verification to the ISHRS HQ and a special code will be emailed to you within 48-hours. This code will allow you to register for the meeting. If the aforementioned step has been completed and you have not received your special code within 48-hours, please call the ISHRS Headquarters.

Non-Physicians (Surgical Assistants, Office Managers, Office Staff, Clinic Directors, Marketing/Webmasters, or Spouse/Guests):
Regardless of your ISHRS membership status, your employing physician or Physician Member spouse
must
first register for the Annual Meeting and fax or email a
Letter of Attestation
on your behalf before you may register. Once your employing physician has registered and submitted your Letter of Attestation to the ISHRS HQ, they will receive a special code which you can utilize to register yourself. If the before mentioned steps have been completed and you have not received your special code within 48-hours, please call the ISHRS Headquarters.
