Gala Dinner
We are pleased to host a gala dinner for all seminar attendees on Friday, June 25, 2010, at 7.00 PM at Kamolthip Ballroom, Montien Riverside Hotel. The dinner will be both an opportunity to review the first day of the seminar and to converse with other seminar attendees. The spouse can join the Gala Dinner with extra $40 USD. You may pay for spouse during the online registration process.
Dinner Show, Siam Niramit
 The registration also includes this dinner show on Saturday June 26, 2010. Siam Niramit is a world-class performance of Thailand's arts and cultural heritage. This must-see spectacular is performed on the gigantic stage now listed in the Guinness World Records, featuring over 150 performers with as many as 500 costumes. On top  of this, enhanced special effects with the world's most advanced technology are used to produce a stimulating, very realistic and inspiring experience. This is a Journey to the Enchanted Kingdom of Thailand that no one can miss. The bus pick up time is at 5.30 PM, please wait at the hotel lobby of Montien Riverside Hotel. The spouse can join with extra $55 USD. You may pay for spouse during the online registration process.
Registration
Click the “Register Now” tab to register online. It is highly encouraged to advance register online to save your spot. Only 60 attendees will be permitted. If you wait to register onsite, you will only be allowed if there are still available slots for up to 60 attendees. In addition, only cash payments in USD will be accepted during onsite registration, and credit card payments will not be accepted onsite.
Weather
Temperature in June is average around 28-30 degrees Celsius (82.4-86 degrees Fahrenheit), and it is the rainy season. It is recommended that you bring an umbrella.
Meeting Attire
Attendees should bring their own scrub suits for the live surgery workshop. Attendees should wear business attire for the morning lectures and bring along their scrubs in a bag, and then they will change into their scrub suit at the hospital. Caps, masks and shoe covers will be provided at the hospital. Attire for the gala dinner is semi-formal. Attire for the Siam Niramit dinner show is casual. You may also want to pack an umbrella as this is the rainy season.
Official Language
The official language of the Workshop is English. Simultaneous interpretation will not be provided.
Submit an Abstract for the Free Paper Session
Attendees are welcome to submit an abstract for a free paper oral presentation at the workshop. There are limited slots, so please submit your abstract early to Dr. Damkerng Pathomvanich at: path_d@hotmail.com . Deadline to submit an abstract is March 31, 2010, and should not exceed 250 words. Each selected abstract will be 10 minutes total = 7-8’ presentation + 2-3’ Q&A.
Video, Photography & Audio Recording Policy
Video and audio recording, and photography are prohibited in the Workshop. The only exceptions to the photography policy are photos taken by ISHRS and Dr. Pathomvanich’s staff, for the purposes to use in ISHRS publications.
Persons with Disabilities
The ISHRS fully complies with the legal requirements of the Americans with Disabilities Act (ADA) and the rules and regulations thereof. If any participant in this educational activity is in need of support services, please notify the Workshop Registrar at 630-262-5399, as soon as possible, and preferably before May 25, 2010.
Cancellation/Refund Policy
Registration fees, less a $100 USD administration fee, will be refunded upon written notice to the Workshop Registrar received on or before May 25, 2010. After May 25, there will be no refund of fees for cancellation or for lack of attendance without notification. It is your responsibility to ensure that your cancellation request has been received by the Workshop Registrar. No shows who have not pre-paid will be invoiced for the total registration fee.
