ISHRS Member($150.00 USD per Webinar) For the individual who is a current member of the ISHRS. ISHRS Member Pending($165.00 USD per Webinar) For the individual who has applied for membership. A complete application with fee must be received at the ISHRS headquarters office to be eligible for this rate. Non ISHRS Member*($180.00 USD per Webinar) For the individual who is not a current member and has not applied for membership. Non-Members may include physicians, residents or training fellows, surgical assistants, office managers, office staff, or clinic directors/owners. *Registration verification is required for all non-members and must accompany registration form. Acceptable forms of verification are as follows: For the Non-Member Physician: a statement on the physician’s letterhead, a copy of the physician’s medical certificate or license, or, a letter from a current ISHRS physician member attesting to the credentials as a physician. For the Non-Member Physician Resident: a letter of verification from the residency program director. For the Training Fellow: a letter of verification from the fellowship training program director. For the Non-Member Surgical Assistant, Office Staff, Office Manager, or Clinic Director/Owner: a letter attesting to current employment by a physician on the physician’s letterhead, or a letter from a current ISHRS physician member attesting to the credentials of the individual.
