The Cicatricial Alopecia Research Foundation together with Dr. Victoria Barbosa and the CARF Chicago Patient Support Group invite you and your family to attend a two-day educational conference.  Learn more about the latest in research, treatment options, and network with other patients, loved ones, and physicians.

Registration Fee Includes

Conference sessions on Saturday and Sunday, handout materials, coffee breaks, and one ticket to the Saturday CARF Dinner.  There is an extra, nominal fee to attend the Friday evening Reception.  There is not an additional fee, but registration is required, to attend the Sunday morning Yoga and/or Sunday morning Express Yourself Support Groups.

 

Official Language
The official language of the Workshop is English. Simultaneous interpretation will not be provided.

Meeting Attire
Educational Sessions: casual attire.
Friday Evening Reception: casual attire, however, the following are not permitted at Lucky Strike: athletic wear, sweats or sports jerseys, motorcycle colors, excessively baggy clothing, sleeveless t-shirts, plain white t-shirts (short or long sleeve), construction boots, headgear, chains, ripped or soiled clothing. 
Saturday Dinner
: business causal.

Sunday Yoga Session: yoga attire, athletic/comfortable clothing.  Bring your own yoga mat.

Weather
Chicago averages a daily maximum temperature for April that's between 55 and 63 degrees Fahrenheit (13 to 17 degrees Celsius). The minimum temperature usually falls between 36 and 41 °F (2 to 5 °C). It is recommended to bring a jacket and umbrella, as Chicago weather can be unpredictable.  In addition, it is recommended to bring a sweater or light jacket for the educational sessions because hotel conference rooms can get chilly at times.

Confirmation and Cancellation/Refund Policy

Upon receipt of your registration, we will send you a confirmation. If you do not receive an e-mailed confirmation letter, please contact us.  Cancellations received by March 25, 2014, will be refunded, less a $25 per person administration fee. Cancellations are not refundable after March 25, 2014.

Video & Photography Policy
Video and photography are prohibited at the conference. The only exception is photos taken by CARF appointed volunteers.

Cellular Phone Policy
We kindly ask that cellular phones are turned to vibrate during the education sessions and calls are taken in the hallway.

 

Persons with Disabilities
CARF fully complies with the legal requirements of the Americans with Disabilities Act (ADA) and the rules and regulations thereof. If any participant in this educational activity is in need of support services, please notify the Conference Registrar as soon as possible.

 

Conference Volunteers
